Antibody-drug conjugates are like smart bombs, ji — they use an antibody to carry a poison straight to the cancer cell. Trastuzumab deruxtecan worked for HER2-low because it finds even faint HER2 signals and delivers a strong chemo, but that same poison can sometimes spill and damage lung tissue. I know this too well because my scans show new ground-glass opacities, the price for shrinking tumours in my liver. The obvious answer is to celebrate a new treatment, but my oxygen cylinder hissing by the bed reminds me the bomb doesn't always know where the enemy ends.
