ACDs are like smart bombs — antibody finds the cancer cell, linker carries the chemo warhead inside. Trastuzumab deruxtecan's linker is *vhodlem* because it's stable in blood but breaks apart inside the tumour, which is why it works even for low HER2. But sometimes that potent drug gets loose in the lungs, maybe from early linker cleavage or off-target cells.
