TB treatment is long because those drugs attack the bacteria in different, slow ways—isoniazid kills actively dividing ones, rifampicin stops protein making, pyrazinamide works in acidic zones, and ethambutol blocks cell wall building. But from my lab here, I see the obvious answer isn't good enough anymore; this long course fails too many people. That's why I work on new candidates that can kill dormant bacteria faster, because I've seen reports from Bastar where entire courses are wasted when power cuts stop the local clinic's fridge, ruining these very drugs.
